The Specialist Community Learning Disability Service offers support to people with learning disabilities and their carers in Sheffield.

We aim to help service users live a happy and healthy life, through integrated care provided in your community.

We offer both a standard and an enhanced care pathway to provide care based on your specific needs:

  • The standard offer includes core aspects of need which support day to day living for people with learning disabilities
  • The enhanced pathway provides a high priority response when more intensive support is required, with the aim of avoiding hospital admissions for people with learning disabilities

We have bases at both Grenoside Grange and Firshill Rise.

This service was launched in 2025 to offer an integrated and streamlined service, replacing three teams which were previously running separately. This service model was coproduced with people with lived experience of using learning disability services, carers, SHSC staff and wider NHS and social care experts.

How do I access the service?

We have one entry route for every referral or query. Anybody can complete a referral, either by filling in a form or by calling the service. 

The referrals will be reviewed in the daily central hub by a multidisciplinary team. The team will assess each referral and, if appropriate, will allocate it to the care pathways most suitable to meet your needs.

The service is open to people aged 18 and above who live in Sheffield. You can read the full eligibility and selection criteria here.

We also offer guidance around reasonable adjustments to support people with learning disabilities to access mainstream health services. This is part of our ‘greenlight for mental health’ commitment to work jointly with other services

Learning disabilities and vaccinations

If you have a learning disability you can get a free flu vaccination every year. You should also get your COVID-19 vaccine. If you need extra help, call the team on 0114 2261562.
