Homeless Assessment and Support Team (HAST)

Description of the service

The Homeless Assessment and Support Team provides access to healthcare and support services for homeless people in Sheffield with mental health problems. It aims to provide an assessment service and, where appropriate, offer treatment and give support in establishing tenancies, and to enable those users with enduring mental health problems to access and engage with mainstream mental health services.

The service has close links with Housing Solutions and other interim accommodation providers in the City.

Most contacts take place in the community, though there are facilities for users to be seen in the team base as well.

The team has 3 posts: one community mental health nurse; one social worker; and one social worker partly funded by the Department of Health Rough Sleepers Initiative. The team also has input (one session a week) from a Consultant Psychiatrist and has part-time admin.

Hanover Medical Centre.

How is the service accessed?

People are referred in to the service by a wide range of agencies. Users can also self-refer. Referral forms are available by contacting the team's office.
